Declaration: I understand very clearly that my health is in good condition enough to join Top Class Football Academy's training and games held at both Hong Kong (means HKSAR) and outside Hong Kong (means the rest of the World). I also understand very clearly that I am capable of participating in any training or games provided by Top Class Football Academy. Whenever any injuries, arising from accidental crashes each other or one another, or due to bad physical conditions personally, that causes any individual injured or even death. I declare that Top Class Football Academy has no responsibility on the accident or death.
